      Name recorded as Forest [sic] Harold Waller, b. July 12 1907 in Industrial [located in Doddridge Co on outskirts of Salem, which is in Harrison Co], WV, son of Frank & Fannie Waller. Source of info listed as Xenia E. Bond, possibly a physician. Although he understandably later assumed the more common spelling of "Forrest" for his first name, there is no explanation for also changing his middle name Harold to John, as found in his marriage record and elsewhere. Through other records, it was determined that his father's name was actually Thomas Frank Waller.
